Transitional Care and Diversion Interventions
The Transitional Care and Diversion Interventions Workgroup met through 2021 as a group of community stakeholders working to help the North Central region implement effective evidence-based practices in the areas of transitional care and diversion intervention:
- Transitional Care – Improving transitional care services to reduce avoidable hospital utilization and ensure beneficiaries are getting the right care in the right place
- Diversion Interventions – Implementing diversion strategies to promote more appropriate use of emergency care services and person-centered care through increased access to primary care and social services, especially for medically underserved populations.
