Transitional Care and Diversion Interventions

The Transitional Care and Diversion Interventions Workgroup is a group of community stakeholders that will ensure that the North Central region implements effective evidence based practices in the areas of transitional care and diversion intervention.   Those two areas are defined as follows:

  • Transitional Care – improve transitional care services to reduce avoidable hospital utilization and ensure beneficiaries are getting the right care in the right place
  • Diversion Interventions – Implement diversion strategies to promote more appropriate use of emergency care services and person-centered care through increased access to primary care and social services, especially for medically underserved populations.

The Transitional Care and Diversion Intervention Workgroup meets the 4th Thursday of every month from 10AM – 11:30 AM.  The workgroup makes every effort to move their meetings throughout the 4 counties (Chelan, Douglas, Grant, and Okanogan).
